50 year-old woman with previous right breast cancer/radiation, left with a fair amount of asymmetry.  New left sided carcinoma (+ relatively widespread DCIS) found; elected to undergo bilateral mastectomies.  Due to her fitness it was felt she didn't have adequate abdominal tissue to allow for TRAM flap reconstructions, and due to her anticipated thin remaining breast tissues, it was felt that expander/implant reconstruction would lead to unacceptable implant visibility. 

Therefore it was recommended she undergo latissimus dorsi (back skin/muscle) rotation flap reconstructions over moderate sized (300cc) gel implants as an immediate, single stage reconstruction.  These photos are from only one week post-op, and demonstrate the excellent breast mound volume/contour symmetry that can be achieved in a single operation.
